About The Position

Amazing Care Home Health Services is seeking a Speech Language Pathologist to join their team in the Ogden area. The Speech Language Pathologist (SLP) will implement treatment plans to assist clients of all ages with cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning by developing and administering speech therapy services in the home and community settings. This role involves providing compassionate and high-quality care, performing evaluations, interpreting assessment results, developing plans of care with skilled interventions and goals, and creating and implementing speech and language treatment plans in conjunction with the physician. The SLP will assist pediatric and adult patients in developing or regaining cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning, thereby improving their level of independence and quality of life. Additionally, the position requires coordinating care with referring physicians and other healthcare organizations, instructing clients, families/caregivers, and other healthcare personnel on the treatment plan, identifying client and family/caregiver needs for other services and referring as appropriate, preparing and submitting documentation per organization policy, and maintaining clinical competency in the theory and practice of speech-language pathology.

Requirements

  • A master's or doctoral degree in speech-language pathology (SLP)
  • ASHA Certification or documentation of passing PRAXIS exam and completion of a Clinical Fellow/Externship
  • Speech-Language Pathologist (SLP) License in the state of Utah
  • Active BLS certification
  • Valid driver's license and auto insurance

Nice To Haves

  • A minimum of 1-year clinical experience in Speech Language Pathology is preferred, but not required

Responsibilities

  • Implement treatment plans to assist clients of all ages with cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning by developing and administering speech therapy services in the home and community settings.

  • Coordinates care with referring physician and other health care organizations and personnel to ensure safe, appropriate, and effective care for the client.
  • Instructs client, family/caregiver, and other organization health care personnel in the client's treatment plan as indicated.
  • Identifies client and family/caregiver needs for other services and refers as appropriate.
  • Prepares and submits documentation per organization policy.
  • Maintains clinical competency in the theory and practice of speech-language pathology.
