About The Position
Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) for a PRN position in the Orange Park, FL area. This role offers flexibility, the opportunity for meaningful patient relationships, and the ability to control income through a pay-per-visit model. The position involves 1:1 patient care in the home, with territories designed to minimize drive time. Requirements
- Active, unencumbered Florida Speech-Language Pathologist license
- Valid driver’s license, reliable transportation, and auto insurance
Nice To Haves
- Home health experience preferred but not required
- OASIS/Medicare documentation experience preferred
Responsibilities
- Conduct speech, language, cognitive, and swallowing evaluations
- Develop individualized treatment plans based on physician orders
- Provide therapy for communication, cognition, and dysphagia
- Educate patients and caregivers to improve functional outcomes
- Collaborate with physicians and interdisciplinary teams
- Complete OASIS documentation as required
- Document visits using streamlined, AI-assisted technology
