SLP - Home Health - Montgomery County
About The Position
Horizon Home Healthcare LLC is seeking a licensed Speech-Language Pathologist (SLP) to provide high-quality speech therapy services in patients' homes in Montgomery County, Ohio, and surrounding areas. This part-time/PRN role offers flexibility, autonomy, and the opportunity for one-on-one patient care, focusing on making a meaningful difference in patients' lives. The SLP will evaluate and treat speech, language, cognitive, voice, and swallowing disorders, develop individualized treatment plans, educate patients and caregivers, complete documentation, and collaborate with other healthcare professionals while maintaining compliance with home health regulations.
Requirements
- Current Ohio SLP license
- Current CPR and/or BLS certification
- Valid driver's license, reliable transportation, and auto insurance
- Excellent communication and organizational skills
Nice To Haves
- Home health experience is preferred
Responsibilities
- Evaluate and treat speech, language, cognitive, voice, and swallowing disorders
- Develop individualized treatment plans based on patient goals
- Educate patients and caregivers on treatment strategies and home programs
- Complete timely and accurate clinical documentation
- Collaborate with physicians, nurses, therapists and other disciplines of the healthcare team
- Maintain compliance with state and federal home health regulations
