Speech Language Pathologist – Dubuque, IA
About The Position
FOX Rehabilitation is expanding in Dubuque, IA, and is seeking dedicated, licensed Speech Language Pathologists to provide exceptional one-on-one care to older adults in their homes or within Senior Living Communities. This role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model. It is ideal for SLPs who value autonomy, flexibility, and meaningful patient outcomes, with the ability to manage their own schedules and build strong patient relationships. The role may also incorporate telehealth and virtual care to enhance flexibility, continuity of care, and patient reach. This is a high-impact SLP position aimed at positively influencing the lives of the communities served.
Requirements
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Current CPR certification
Responsibilities
- Provide one-on-one Speech Language Pathology to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
- Document effectively within FOX’s EMR system in accordance with regulatory standards
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
- Provide telehealth / virtual care when appropriate to extend access and flexibility
