About The Position

Heritage Health Care is seeking a full-time Speech Language Pathologist (SLP) to join their team. This role involves performing evaluations and treatments to ensure the highest degree of quality care. The SLP will be responsible for writing clear, realistic, and measurable goals tailored to individual patient needs, while treating patients with respect, dignity, and privacy. The position requires motivating patients to participate in therapy, using creative interventions based on patient interests and needs to achieve positive outcomes, and participating as an interdisciplinary team member to determine patient needs and plan of care. Accurate and timely documentation of patient progress is essential, as is the effective utilization of the company's computer system in accordance with company policy. An active AHA CPR certification is required for Outpatient positions.

Requirements

Active AHA (American Heart Association) CPR certification (if applying for an Outpatient position).

Responsibilities

Perform evaluations and treatments ensuring the highest degree of quality care.
Write clear, realistic, measurable goals specific to individual patient needs.
Treat patients with respect, dignity, and privacy.
Motivate patients to participate in therapy treatment.
Use creative therapy interventions specific to the patients’ interests and needs to obtain positive patient outcomes and progress.
Participate as an interdisciplinary team member to determine potential patient needs and plan of care.
Accurately complete documentation of qualitative and quantitative data of patient progress within designated time period.
Utilize the computer system effectively, adhering to company policy.
