About The Position
Concierge Home Care is hiring a Home Health Speech-Language Pathologist (SLP) in Crestview and the surrounding area. This role offers flexibility, meaningful patient relationships, and the ability to control income. The coverage area is Crestview, FL 32536, with territories designed to minimize drive time through clustered scheduling. The position emphasizes 1:1 patient care in the home, AI-assisted charting, and supportive leadership.
Requirements
- Active, unencumbered Florida Speech-Language Pathologist license
- Valid driver’s license, reliable transportation, and auto insurance
- Employment is contingent upon successful completion of an AHCA Level 2 background screening.
Nice To Haves
- Home health experience preferred but not required
- OASIS/Medicare documentation experience preferred
Responsibilities
- Conduct speech, language, cognitive, and swallowing evaluations
- Develop individualized treatment plans based on physician orders
- Provide therapy for communication, cognition, and dysphagia
- Educate patients and caregivers to improve functional outcomes
- Collaborate with physicians and interdisciplinary teams
- Complete OASIS documentation as required
- Document visits using streamlined, AI-assisted technology
