Requirements
- Active Minnesota Speech-Language Pathologist (SLP) license.
- Passion for helping patients regain independence and confidence.
- Excellent communication and clinical reasoning skills.
- Valid driver's license and reliable transportation.
Nice To Haves
- Home health experience is welcomed but not required.
Responsibilities
- Perform comprehensive speech, language, cognitive, and swallowing evaluations in patients' homes.
- Develop individualized treatment plans focused on meaningful functional outcomes.
- Provide evidence-based interventions for communication, cognition, and dysphagia.
- Educate patients, caregivers, and family members to promote long-term success.
- Collaborate closely with physicians, nurses, therapists, and the interdisciplinary care team.
- Complete timely and accurate documentation using our electronic medical record.
