About The Position

H2 Health Prime Living is seeking a Speech Language Pathologist for PRN (4-10 visits/week) to provide 1:1 patient care in senior living communities. This role involves evaluating and treating a primarily geriatric caseload with a focus on dysphagia, cognitive impairment, neurologic disorders, and problem-solving programs. The ideal candidate can work independently and as part of a team, making sound decisions based on patient needs. Requirements

• A graduate of an accredited Speech Language Pathology program.
• Experience with dysphagia is required.
• Active CCC-SLP state license.

Nice To Haves

• Prior experience in outpatient therapy is a plus.
