About The Position

Concord Post-Acute is seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our rehabilitation team on a per diem basis. We are dedicated to helping our residents achieve their highest level of independence through individualized therapy programs and collaborative care. Requirements

• Current California Speech-Language Pathologist license required
• Current CPR certification preferred
• Strong clinical assessment and treatment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and collaboratively within a multidisciplinary team

Nice To Haves

• Certificate of Clinical Competence (CCC-SLP) preferred
• Skilled nursing, post-acute, or rehabilitation experience preferred

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide speech, language, cognitive, communication, and swallowing therapy services
• Assess and manage dysphagia and recommend appropriate dietary modifications
• Maintain accurate and timely clinical documentation
• Collaborate with interdisciplinary team members to coordinate resident care
• Educate residents, families, and caregivers regarding treatment goals and progress
• Participate in care planning meetings and discharge planning as needed
• Ensure compliance with all regulatory, ethical, and facility standards
