About The Position

HealthPro Heritage has a great PRN Speech Language Pathologist (SLP) Opportunity in Trumbull, CT. The role involves providing Outpatient services to residents of an Award-winning 93-bed assisted living and 27-bed memory care community, dedicated to keeping seniors connected to the people, passions and things that matter most to them. Care is provided in an onsite gym or in the patient's room. Requirements

  • Degree in Speech Language Pathology from an accredited institution.
  • Valid state licensure as a Speech Language Pathologist, or license eligible in the state of Connecticut.
  • Certification from the American Speech-Language-Hearing Association (ASHA) (CCC-SLP).
  • Strong assessment, diagnostic, and therapeutic abilities.
  • Excellent communication and interpersonal skills.
  • Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems.
  • Proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

  • Conduct comprehensive evaluations of speech, language, voice, and swallowing abilities using standardized tests and clinical observations.
  • Develop accurate diagnoses based on assessment results.
  • Implement evidence-based therapies for speech, language, voice, and swallowing disorders.
  • Monitor and adjust treatment plans.
  • Educate and support patients and families on disorders, treatment strategies, and daily communication improvement.
  • Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
  • Work with healthcare providers to ensure effective patient care.
  • Participate in multidisciplinary team meetings as needed.
