Assess speech, language, cognitiveâcommunication, and swallowing abilities in the home environment Identify functional strengths and challenges that impact daily living Develop tailored treatment plans based on evaluation findings and patientâcentered goals Provide individualized, targeted therapy to improve communication, cognitive function, and swallowing safety Integrate therapy strategies into clients’ natural routines and environments Address communication, cognition, and dysphagia needs with a wholeâperson approach Educate patients and caregivers on strategies to support communication and swallowing at home Coordinate care with an interdisciplinary team including nurses, PTs, OTs, and care coordinator
