Speech Language Pathologist (SLP) - Outpatient

H2 Health is seeking a Speech-Language Pathologist to join our outpatient team in Bluefield, Virginia. In this role, you'll work with a diverse outpatient population while providing individualized, evidence-based therapy services. You'll collaborate with Physical Therapists, Occupational Therapists, physicians, caregivers, and other healthcare professionals to help patients maximize communication, cognition, swallowing function, and independence.
Requirements
- Master's degree in Speech-Language Pathology from an accredited program.
- Current Virginia Speech-Language Pathologist license or eligibility for licensure.
- CPR certification or willingness to obtain as required.
- Strong communication and interpersonal skills.
- Commitment to providing high-quality, patient-centered care.
Nice To Haves
- Certificate of Clinical Competence (CCC-SLP) preferred or eligibility to complete Clinical Fellowship Year (CFY).
- New graduates and Clinical Fellows are encouraged to apply.
Responsibilities
- Perform comprehensive speech-language evaluations and develop individualized plans of care.
- Assess and treat communication, language, cognitive-linguistic, voice, fluency, and swallowing disorders.
- Deliver evidence-based interventions designed to improve communication skills, cognition, safety, and quality of life.
- Collaborate with patients, caregivers, physicians, and interdisciplinary team members to achieve optimal outcomes.
- Educate patients and families on treatment strategies and home programs.
- Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
- Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
