About The Position
McClure Post Acute is seeking a compassionate, knowledgeable, and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis. If you're passionate about helping residents improve their communication, cognition, and swallowing abilities while working in a collaborative post-acute environment, we'd love to hear from you. As a valued member of our interdisciplinary team, you will evaluate, develop, and implement individualized treatment plans that promote functional independence and improve quality of life for our residents.
Requirements
- Master's Degree in Speech-Language Pathology from an accredited program.
- Current, unrestricted California Speech-Language Pathologist (SLP) license.
- Excellent communication, documentation, and interpersonal skills.
- Strong commitment to providing compassionate, resident-centered care.
Nice To Haves
- Certificate of Clinical Competence (CCC-SLP) preferred.
- Clinical Fellows (CFY) are encouraged to apply if eligible for supervision.
- Skilled nursing or post-acute care experience is preferred but not required.
Responsibilities
- Evaluate residents to determine speech, language, cognitive, voice, and swallowing needs.
- Develop and implement individualized treatment plans based on physician orders and resident goals.
- Provide evidence-based therapy to improve communication, cognition, swallowing safety, and overall functional outcomes.
- Monitor resident progress and modify treatment plans as appropriate.
- Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and family members to ensure coordinated care.
- Maintain timely, accurate, and compliant clinical documentation.
- Participate in interdisciplinary care plan meetings, rehabilitation conferences, and discharge planning.
- Educate residents, families, and caregivers on therapy techniques, home programs, and aspiration precautions.
- Assist with staff education and provide clinical in-services as needed.
- Supervise Clinical Fellows (CFY) and Speech-Language Pathology students when applicable.
- Maintain compliance with all facility, state, federal, and professional practice standards.
