About The Position

Discovery at Home is seeking a Speech Language Pathologist in Cape Coral, FL. This is a part-time or PRN position, with up to 3 days per week. The Speech Language Pathologist will primarily conduct home visits with the potential for some caseload within assisted and independent living communities. The patient population is mainly geriatric, with most cases under Medicare Part A Home Health, and some under Part B.

Requirements

Graduation from a Speech-Language Pathologist program.
State license as a Speech-Language Pathologist (eligible for FL state licensure).
Valid driver’s license and reliable transportation.
Current CPR certification.
Minimum of 1 year of prior home care experience.
Completion of fingerprinting and background screening through the AHCA Clearinghouse.

Responsibilities

Conduct home visits with geriatric patients.
Manage caseloads within assisted and independent living communities.
Provide care under Medicare Part A Home Health and Part B.
Determine own caseload and work schedule.
Complete Level 2 background screening.
