About The Position

As a Speech Language Pathologist at VHA, the role involves assessing dysphagia and communication disorders in adults and geriatric clients, providing consultation, recommendations, and evidence-based interventions in their homes. The position requires coordinated care delivery through communication with the client care team, problem-solving for client independence, and promoting VHA’s core values. A key aspect of this role is providing advice, mentoring, consultation, and expertise in Speech Language Pathology. This includes conducting joint visits, supporting colleagues with challenging cases, providing clinical orientation, and mentoring new staff. The role also involves identifying and participating in best practice initiatives, meeting with the Supervisor of Rehab Best Practice, and acting as a resource for professional standards, policies, and procedures.

Requirements

  • Master’s Degree or equivalent in Speech Language Pathology.
  • Membership in good standing with the College of Audiologists and Speech Language Pathologists of Ontario.
  • Eligible for membership in the Ontario Association of Speech Language Pathologists and Audiologists.
  • Minimum of 3 years’ experience in community health preferred.
  • Valid Driver’s License with access to a vehicle.
  • Compassionate and empathetic.
  • Self-driven with ability to work independently.
  • Skilled in caseload management.
  • Skilled at electronic documentation.
  • Able to provide virtual care.

Responsibilities

  • Assessing and diagnosing clients through the selection and administration of standardized and non-standardized tests.
  • Devising and implementing individualized goal-directed treatment programs based on case history, assessment results, current professional practices, and College standards.
  • Monitoring client progress and evaluating treatment and management on an ongoing basis.
  • Counseling and educating clients, caregivers, and family members regarding communication deficits and teaching self-management strategies.
  • Discussing clinically significant information with physicians or other appropriate members of the health care team.
  • Assisting in client discharge planning in conjunction with the client, family members, and other health care team members.
  • Maintaining accurate and comprehensive written documentation of client care in accordance with legal, professional, and VHA standards.
  • Maintaining knowledge of current developments in Speech Language Pathology and Community Services.
  • Adhering to the code of ethics of the profession and maintaining standards of practice as defined by the Ontario College of Audiologists and Speech Language Pathologists.
  • Providing advice, mentoring, consultation, and expertise related to the Speech Language Pathology profession.
  • Providing joint visits with other Speech Language Pathologists and Service Providers to give input for performance reviews.
  • Supporting Speech Language Pathologists and Service Providers facing clinically challenging situations.
  • Providing clinical orientation for new Speech Language Pathologists and Service Providers or those changing clinical areas.
  • Mentoring groups for new graduates or those new to the community setting.
  • Identifying, promoting, and participating in best practice initiatives.
  • Meeting regularly with the Supervisor, Rehab Best Practice.
  • Acting as a resource in the implementation and interpretation of discipline-specific professional standards of practice.
  • Contributing to the development of standards of practice, clinical policies, and procedures relevant to the profession.
