Specialty RN - Wound Care - Per Diem - Days
About The Position
Provides direct and indirect patient care in the office setting to patients, families, and caregivers within the Medical Group practice. Utilizes the nursing process consistent with acceptable standards of the profession and MVHS Medical Group. Delivers high-quality, patient-centered care with initiative, flexibility, and responsibility, requiring minimal administrative supervision. Determines care priorities based on physical and psychosocial needs, as well as factors influencing patient flow. Communicates with outpatient clinic physicians and NP/PAs regarding changes in patient status, symptomatology, and diagnostic study results. Possesses the ability to respond quickly and accurately to changes in condition or response to treatment.
Requirements
- Graduate of an accredited school of nursing or nursing program.
- Current New York State Registered Nursing registration.
- BLS Certification.
- Minimum one year of nursing experience.
- Basic computer and EMR knowledge.
- Excellent communication and interpersonal skills.
- Ability to assimilate information from many different sources.
- Must be able to communicate in English both verbally and in writing.
Nice To Haves
- BSN from accredited nursing program
- Experience in a specialty medical office.
- Certifications applicable to the assigned office’s specialty.
Responsibilities
- Support the PCP in the delivery of care by gathering and documenting relevant clinical data, as appropriate and per patient need.
- Participate in the daily huddle to identify individual patient needs for upcoming appointments.
- Act as a clinical liaison between patient and provider for questions, concerns, and testing results.
- Communicate effectively with other members of the care team in coordinating the individualized plan of care.
- Support PCP during in-office procedures and examinations.
- Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management).
- Perform duties within scope of practice to the highest level of licensure.
- Provide direct support and guidance to other members of the care team as appropriate.
- Perform assessment data collection on all patients utilizing tools/forms in EMR.
- Adequately assess and reassess pain. Educate the patient and family regarding pain management.
- Provide age-appropriate care to patients in the community setting.
- Consistently document procedures completed in the EMR, to include medications administered and follow-up assessments.
- Deliver medication utilizing the five (5) rights and observes patient for desired or untoward effects.
- Maintain current knowledge of medications and their correct administration.
- Establish priorities of patient care based on needs and office resources.
- Demonstrate the ability to assist providers with procedures and perform services requiring technical and manual skill.
- Perform other duties as required.
