About The Position

St. Luke's Home Delivery Pharmacy is seeking a full-time Specialty Pharmacist to support the continued growth of our Specialty Pharmacy program. This pharmacist will provide comprehensive pharmaceutical care, including specialty patient assessment and onboarding, medication therapy management, clinical interventions, patient education, and product verification. The position plays a critical role in supporting specialty program growth, Medication Access and Specialty Optimization Team (MSOT) initiatives, integrated care services, and longitudinal patient management.

Requirements

  • Degree from an accredited School of Pharmacy (PharmD or equivalent).
  • Licensed Pharmacist in the State of Idaho or eligible for licensure.
  • Current Basic Life Support (BLS) certification through the American Heart Association, American Red Cross, or American Safety and Health Institute.
  • Strong communication, clinical assessment, and patient education skills.

Nice To Haves

  • Specialty pharmacy, ambulatory care, home delivery, or health-system pharmacy experience.
  • Experience managing complex chronic disease states and specialty medications.
  • Board certification or specialty pharmacy certification (if applicable).

Responsibilities

  • Conduct specialty patient assessments and onboarding activities.
  • Review and verify specialty medication orders for appropriateness, safety, and effectiveness.
  • Provide medication counseling, education, and ongoing clinical support to patients and caregivers.
  • Collaborate with providers, nurses, and care teams to optimize medication therapy and patient outcomes.
  • Perform clinical interventions and document care activities according to organizational and regulatory standards.
  • Answer patient and provider medication-related questions.
  • Interpret prescriptions, dispense medications, and oversee specialty pharmacy workflow.
  • Supervise and support pharmacy technicians, interns, residents, and support staff.
  • Maintain accurate pharmacy records, documentation, and regulatory compliance.
  • Administer immunizations and medications as appropriate.
  • Serve as a mentor and resource for pharmacy staff and learners.
  • Participate in quality improvement and operational initiatives supporting Specialty Pharmacy growth and patient care excellence.
