About The Position

Under the direction of the Clinical Pharmacy Manager, the Specialty Pharmacist serves as the primary pharmacist supporting Saban Community Clinic's Specialty Pharmacy Program and centralized medication refill services. The role involves coordinating specialty medication management, patient onboarding, education, care planning, and reassessments. The pharmacist will monitor medication adherence, therapeutic response, and adverse effects, collaborating with providers to optimize therapy. Responsibilities also include processing medication refill requests, identifying and communicating medication-related concerns, assisting with prescription clarifications and prior authorizations, and providing coverage within the retail pharmacy as needed. The role also entails verifying and dispensing prescriptions, providing patient counseling, supervising pharmacy technicians, and participating in inventory management and quality assurance. Additionally, the Specialty Pharmacist will support the growth of the Specialty Pharmacy Program through program development and workflow optimization.

Requirements

  • Doctor of Pharmacy (PharmD) degree.
  • Current California Pharmacist license in good standing.
  • Minimum of one (1) year of pharmacist experience in specialty pharmacy, ambulatory care, or an integrated healthcare setting required.
  • Strong organizational, communication, critical thinking, and problem-solving skills with attention to detail.
  • Ability to work independently and collaboratively within an interdisciplinary healthcare team.
  • Able to communicate professionally and effectively with patients, providers, pharmacy staff, and other healthcare team members.

  • California Advanced Practice Pharmacist (APh) license

Responsibilities

  • Serve as the primary pharmacist for the Specialty Pharmacy Program.
  • Coordinate specialty medication management from referral through ongoing follow-up.
  • Complete patient onboarding, education, care planning, and reassessments.
  • Monitor medication adherence, persistence, therapeutic response, adverse effects, and medication-related problems.
  • Collaborate with providers to optimize specialty medication therapy.
  • Maintain accurate documentation in the electronic health record and pharmacy systems.
  • Review, evaluate, and process medication refill requests by approving, modifying, or denying requests in accordance with SCC refill protocols; complete timely processing and documentation.
  • Identify medication-related concerns and communicate recommendations to providers.
  • Assist with prescription clarification, electronic prescribing issues, and prior authorizations.
  • Provide pharmacist coverage within the retail pharmacy as assigned to support operational needs, continuity of patient care, and efficient pharmacy workflow.
  • Verify and dispense prescriptions accurately in accordance with state and federal regulations.
  • Provide patient counseling and medication education.
  • Supervise and support pharmacy technicians and verify technician-prepared prescriptions.
  • Participate in inventory management, controlled substance oversight, billing accuracy, medication safety, and quality assurance.
  • Support continued growth of the Specialty Pharmacy Program by participating in program development, workflow optimization, and implementation of new pharmacy services and initiatives.
