About The Position

We are a unique, rapidly expanding primary care provider focused on transforming healthcare for seniors. We are seeking innovative and entrepreneurial-minded individuals with exceptional work ethics and ambition to join our team. As a Specialty Partner, you will be a licensed/Board Certified/Board Eligible professional in a specialty such as urology, otolaryngology, orthopedics, pain management, dermatology, or wound care. You will play a key role in our clinical operations team, providing direct patient care and assessments in various settings including ambulatory health care centers, acute care, nursing homes, skilled nursing facilities (SNF), and home settings. Our mission is to improve the health and well-being of the populations we serve by rescuing seniors from inadequate healthcare.

Requirements

  • Medical degree from an accredited school.
  • Licensed to practice in the state.
  • Minimum of two (2) years of direct work or intern experience.
  • Rigorous medical training and a broad knowledge of various conditions related to specialty, and the ability to treat these conditions effectively.
  • Ability to operate equipment related to specialty, e.g. ultrasound, EKG.
  • Professional but friendly bedside manner with the ability to explain medical conditions to patients in clear and straightforward terms without medical jargon.
  • Thorough knowledge of pharmacological agents used in patient treatment.
  • Excellent analytical skills and attention to detail.
  • Excellent communication, interpersonal and leadership skills.
  • Ability to collaborate with other caregivers including PCPs.
  • Proficient in Microsoft Office Outlook.
  • Ability to learn internal systems such as Dashboard and MyNotes.
  • Ability and willingness to travel within the market up to 75% of the time; may occasionally be asked to travel to other local markets.
  • Spoken and written fluency in English.

Nice To Haves

  • Board certification in specialty preferred.
  • Hospital privileges may be required in some cases.

Responsibilities

  • Provide direct patient care and assessments.
  • Perform geriatric assessments, review medical history, and conduct physical exams.
  • Diagnose and treat conditions within your specialty.
  • Develop and implement patient care plans.
  • Provide health education to patients and their caretakers.
  • Make referrals for case management and conduct follow-up care.
  • Maintain clear and thorough documentation according to ChenMed standards.
  • Participate in conferences and conduct in-depth documentation through progress notes and summaries.
  • Conduct exams and tests to determine likelihood of endocrine gland related issues.
  • Analyze lab results and symptoms to arrive at a diagnosis.
  • Interpret diagnostic testing and determine the nature of symptoms.
  • Explain recommendations and treatment plans to patients and caretakers.
  • Provide clinical advice for daily and long-term management of diseases or conditions.
  • Recommend lifestyle changes regarding diet, hygiene, and exercise.
  • Devise and assign proper courses of treatment.
  • Monitor patient progress during treatment.
  • Follow up with Primary Care Physicians (PCP) and other providers on total patient well-being.
  • Follow up with patients to track their condition.
  • Advise on surgery when necessary.
  • Advise on outside therapy when needed.
  • Collaborate with other specialized physicians to treat and cure certain conditions.
  • Write and renew prescriptions for medications.
  • Stay updated with the newest forms of treatment in your specialty.
  • Update or provide training to center personnel as needed.
  • Attend center meetings as appropriate.
  • Perform other duties as assigned.
