About The Position

This newly created position will manage the Vascular Access Specialty Team consisting of 3 RN's and the Inpatient Wound team of 4 RN's and 3 LVN's. They will also oversee our Safety in Nursing Practice program which includes the HAPI, Falls, and CAUTI/CLABSI prevention committees. Drives quality improvement, patient safety, risk management, and regulatory readiness initiatives by monitoring clinical outcomes, conducting investigations, managing corrective action plans, facilitating committees, and collaborating with multidisciplinary teams. Assists in developing policies, procedures, evidence-based practice guidelines, and performance improvement strategies to reduce risk and enhance patient outcomes.

Requirements

CA RN license
Bachelor’s degree in nursing
LSS tools and Critical thinking for proactive methodologies.
Excellent communication skills, computer skills, and project management skills
Demonstrates a knowledge and understanding of Joint Commission standards California's Nursing Practice Act Applicable sections of Title 22 and 17 Applicable Federal guidelines and regulations including HIPPA Policies and procedures for areas supervised National standards for areas supervised.

Nice To Haves

National certification in wound care, vascular access, and/or quality, Lean Six Sigma training
MSN or Master's Degree
Three years of clinical experience in wound care or vascular access; experience in quality, risk management, or patient safety projects; supervisory or leadership experience

Responsibilities

Has 24-hour responsibility for the quality of care and performance improvement activities in area of supervision.
Demonstrates and utilizes knowledge in the area(s) of practice. Assists staff with clinical assessment and decision-making.
Responds to unexpected staff shortages by maximizing available resources and assisting staff in the prioritization of patient care needs.
Ensures that patient care needs are met in a timely manner and that patient care is coordinated across the continuum of services and sites.
Demonstrates awareness of the needs and preferences of the medical care providers and provides excellent service.
Monitors patient care documentation to ensure the adequacy of patient information and compliance with regulatory standards.
Uses the Statement of Concern process to report patient care issues.
Knows and utilizes the Medical Staff and Kaweah Health chains-of-command.
Assesses and documents the competency of each staff member hired for the areas supervised.
Collaborates with the Educator (if applicable) on the orientation plan for new staff.
Evaluates staff annually. Completes the performance appraisals by the due date.
Identifies performance or education issues through observation, monitoring patient care outcomes and processes, and promptly communicates concerns to the staff member/s.
Uses research findings in the development of policies, procedures, and practice guidelines for patient/resident care. Shares research information with others.
Utilizes HR policies and procedures to ensure Kaweah Health employment standards are met.
Responsible for preparing staff schedules that meet the needs of the patient population and average daily census.
Interviews and hires new employees.
Contributes to staff development by assuring continuing education and in-services are available to staff.
Communicates effectively with key constituencies: Seeks and listens to feedback from key stakeholders: patients, physicians, staff, and other departments/units. Communicates effectively with other patient care leaders. Communicates and cooperates with other Nurse Managers, Bed Manager and Staffing Facilitators to ensure patient care and staffing resources are coordinated.
Actively participates in assigned meetings and projects.
Responds promptly to communication from others. Uses e-mail responsibly.
Practice is guided by the Code for Nurses: Decisions and actions on behalf of patients/residents and staff are determined in an ethical manner. Maintains patient confidentiality within legal and regulatory parameters. Acts as a patient advocate and coaches/mentors staff in advocating for patients. Role models the delivery of patient care in a nonjudgmental and nondiscriminatory manner that preserves patient autonomy, dignity, and rights.
Coordinates education and consultations to patient care departments and other leaders of Quality and Patient Safety, CEO, and Board of Directors, medical staff, department heads and hospital staff.
Works collaboratively with multiple disciplines ensuring effective measures are developed, implemented and maintained to maximize efforts to decrease or eliminate issues identified as non-conforming with CDPH / CMS and other California Regulatory standards.
Participates in Risk Management and Quality and Patient Safety meetings required for event investigation and processes related to policies, procedures and adverse events across the organization in alignment with CMS/CDPH standards.
Organizes and maintains an ongoing mechanism for monitoring action plans and deficiencies for the nursing care division.
Facilitates quality-of-care committees directed towards promoting patient safety issues, collaborating with applicable leaders.
Informs director and other applicable department heads regarding quality issues; feedback to directors at all levels in the effort to eliminate patient safety risks; assists clinical chairs and department heads in designing quality programs within their departments that reflect evidence-based practice.
Assists with patient care investigations, inspections, assisting both the Risk and Quality departments with identified issues, and working directly with senior leadership, management and providers to develop any required action plans for deficiencies.
Champions a variety of patient safety projects assigned by the Chief Nursing Officer and Director of Nursing Practice and Professional Development. Coordinates and facilitates improvement efforts collaboratively with other departments and stakeholders related to key patient safety issues, which can include development of policy/procedures, research, data analysis, or proactive risk assessments (FMEA) for patient safety processes.
Works in collaboration with other organizational leaders to ensure process and policies are aligned with regulatory standards.
Uses effective project management skills. Utilizes change management principles to get results. When facilitating quality teams, manages the work defined in the project charter/project plan/scope to ensure the project moves forward according to the project plan goals and timelines. Maintains project plans/records such that they serve as an archive of project results. Develops and presents written and verbal reports at various points during the lifecycle of assigned projects. Shares this information and works collaboratively with appropriate leadership quality teams.
Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
