Performs outpatient professional nursing consultation and educational services for persons with diabetes of all types and ages in the form of individual appointments, classes, and community events. Assists in the development of curriculum, including the most up-to-date research. Education to be provided includes the necessary lifestyle changes involved with a diabetes diagnosis, which includes but is not limited to; activity, nutrition, social barriers, medications, costs, physical activity, glucose testing, problem-solving, and financial/food assistance. Assists with developing educational policies and procedures. Also responsible for the direction, implementation, and evaluation of the Diabetes Self-management education (DSME) program and its objectives.
