Requirements

Active Illinois PA or APRN/FNP license.
Master’s Degree in Nursing or Physician Assistant studies.
Bilingual in English and Spanish (spoken and written).
Strong clinical assessment and decision-making skills.
Ability to work efficiently in a fast-paced healthcare environment.
Excellent communication and interpersonal skills with a patient-centered approach.

Nice To Haves

2–5 years of experience in Primary Care and Pediatrics preferred.
New graduates are encouraged to apply.
Experience with electronic medical records (EMR/EHR) systems.
Passion for community-focused healthcare and preventive medicine.

Responsibilities

Conduct comprehensive patient assessments including medical histories and physical examinations.
Diagnose and manage acute and chronic health conditions.
Develop and implement individualized treatment plans to support positive patient outcomes.
Work closely with physicians and healthcare teams to coordinate comprehensive patient care.
Prescribe medications and therapies in accordance with clinical protocols and physician collaboration.
Participate in care planning and follow-up management.
Order, perform, and interpret diagnostic tests such as laboratory work, EKGs, and imaging studies.
Perform clinical procedures including: Suturing, Wound care, Injections, Immunizations, Minor in-office treatments.
Educate patients and families on disease prevention, wellness, nutrition, and healthy lifestyle practices.
Provide counseling on family planning, child development, and preventive healthcare.
Promote continuity of care through patient advocacy and education.
Collaborate with medical assistants, nurses, and support staff to ensure efficient clinic operations.
Contribute to a positive, patient-focused work environment.
Maintain accurate and timely medical documentation in compliance with healthcare regulations.
