About The Position

Reporting to the designated Social Work Site Leader and receiving guidance and support from the Professional Practice Leader and/or Research, Education and Practice Coordinator, the Social Worker supports the interdisciplinary program/team in achieving program, patient, and resident goals. Provides therapeutic clinical counselling and conducts comprehensive biopsychosocial assessments for patients and residents. Provides consultative and direct services to patients/residents, families, staff, and community health professionals, including information and support related to community resources. Provides leadership to the interdisciplinary team throughout the discharge planning process and coordinates clinical and case conferences. Maintains accurate patient/resident records and develops and delivers in-services and presentations for staff. Participates in local committees and group meetings and maintains current knowledge of developments and best practices within the Social Work profession.

Requirements

• Master’s degree in Social Work from an accredited university.
• One year of recent, related experience in a designated program area, or an equivalent combination of education, training, and experience.
• Current full registration with the British Columbia College of Social Workers.
• Demonstrated ability to communicate and interact effectively with patients/residents, families, staff, physicians, and other health care providers.
• Knowledge of legislation, government policies, and procedures related to health care.
• Demonstrated ability to gather relevant information and conduct comprehensive biopsychosocial and patient/resident assessments.
• Knowledge of other health care disciplines and their roles in patient/resident care.
• Demonstrated ability to draw out relevant information to better understand the patient/resident’s situation and explore issues beyond the initial concerns.
• Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
• Demonstrated ability to manage conflict and guide others in conflict resolution.
• Ability to work independently, prioritize issues, and manage workload effectively.
• Ability to facilitate and influence group and inter-group meetings, processes, and decision-making.
• Demonstrated ability to provide supervision and teaching to students.
• Ability to participate in research activities.
• Ability to operate related equipment.
• Physical ability to carry out the duties of the position.

Nice To Haves

• A Bachelor of Social Work degree from an accredited School of Social Work, with an equivalent combination of education, training, and experience, may also be considered.

Responsibilities

• Conduct comprehensive social work and psychosocial assessments by interviewing patients/residents and families, gathering relevant social information, and identifying factors that may impact coping and care needs.
• Maintain accurate patient/resident records, including assessments, progress notes, reports and other documentation in accordance with established standards, policies and regulatory requirements.
• Contribute to interdisciplinary discharge planning by assessing care needs, determining appropriate services and collaborating with hospital teams, community partners and other resources for patients/residents with complex needs.
• Provide direct and consultative support to patients/residents, families, staff and community health professionals regarding community resources, financial assistance and access to appropriate services.
• Participate in interdisciplinary care conferences related to patient/resident reviews, admissions, care planning and discharges.
• Support patients, families and care teams in understanding relevant legislative processes, including substitute decision-making, Medical Assistance in Dying, Mental Health Act rights, incapability assessments and adult guardianship.
• Develop and deliver education, in-services, presentations and training for staff in collaboration with members of the interdisciplinary team.
• Facilitate and participate in group meetings, provide clinical counselling and deliver therapeutic and psychosocial education as required.
• Participate in program and practice evaluation, committees, research and quality improvement initiatives to enhance patient/resident care and service delivery.
• Maintain current knowledge of developments in Social Work practice and provide teaching, guidance and supervision to social work students as assigned.
