Candidates will have flexibility in their work schedule to the extent that they can successfully perform the required duties of the position. Experience based rates $26.00-$30.00/hour. At Frederick Living the Social Worker develops, implements and evaluates the social service program for skilled nursing while providing excellent customer service to residents.

Requirements

  • Bachelor’s degree in social work or human services
  • Experience in a Skilled Nursing environment preferred
  • Strong communication skills, both verbal and written
  • Demonstrates respect, integrity, compassion, and excellence
  • Ability to work collaboratively and maintain positive relationships
  • Availability to work occasional odd hours or weekends

Responsibilities

  • Develop and maintain a Social Service Program for Cedarwood that meets all state and federal guidelines.
  • Complete MDS, person-centered care plans and social service documentation in a timely manner.
  • Coordinate hairdresser, vision, dental, podiatry and audiology services.
  • Review and update social service policies and procedures annually and as needed.
  • Assist in issuing Medicare and insurance cut letters, in coordination with RNAC.
  • Serve as the resident advocate including being the Grievance Official for Cedarwood.
  • Coordinate resident and family/representative care conferences in cooperation with the RNAC.
  • Assist with insurance reviews and obtaining authorizations as needed.
  • Perform a variety of services for meeting the psychosocial and personal needs of residents and families, such as assisting with initial adjustment, subsequent changes, trauma informed care assessment, and crisis management.
  • Financial/insurance counseling and assist with the medical assistance application process.
  • Oversee discharge-planning process.
  • Assist NHA and DON with investigation of abuse allegations.
  • Complete and maintain MA 401’s and PASRR’s for collaboration with UMR reviews.
  • Abide by the organization’s code of conduct in the performance of their duties.
  • Perform other duties as required
