About The Position

Provide per diem social work support for patients that are in the hospital. Must be available to work at least two weekend days/month after orientation and onboarding. Practicing under the clinical supervision of an independently licensed clinical social worker (LICSW), is responsible for psychosocial screening, assessment, diagnosis, treatment, and follow-up for patients and families in assigned patient population or service area. Collaborates with physicians, nurses, internal and external case managers, community agencies, and other health care providers to implement safe and effective patient care, support a safe discharge or transition of care.

Requirements

  • Master's degree in Social Work
  • License: Licensed Clinical Social Worker (LCSW)
  • 0-1 years related work experience required.
  • New graduates and out of state MSW prepared Social Workers are expected to obtain a Massachusetts license within 6 months of hire date.
  • Before LCSW Is obtained, must work under the direct clinical supervision and have all notes co-signed by an LICSW.
  • Experience with computer systems required, including web-based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint, One Drive and SharePoint

Responsibilities

  • Provides clinical services to patients and families that address personal and environmental issues to maximize emotional, social, and physical well-being and the appropriate and efficient use of health care resources.
  • Screens and triages patients who request service, are referred, or meet screening criteria for identifying patient needs.
  • Conducts psychosocial assessments and provides services as indicated.
  • Partners with the multidisciplinary team to facilitate continuity of care planning and treatment interventions.
  • Meets productivity standards and expectations as set by the leadership in Social Work and the assigned clinical area.
  • Provides counseling and psychotherapeutic services appropriate to setting.
  • Provides timely and appropriate communication and documentation in the medical record.
  • Provides consultation and education regarding psycho-social issues affecting patient care.
  • Identifies and reports to their manager, potential ethical and risk management issues.
  • Presents and/or discusses clinical work in formal and informal settings, multi-disciplinary and clinical rounds and seminars.
  • Participates in department process improvement and CQI efforts as needed.
  • Participates in clinical supervision as appropriate to level of licensure and professional development.
  • Participates in an environment of idea sharing and feedback.
  • Seeks opportunities for own professional development.
