About The Position

Carver County is hiring a Social Worker in its Home & Community Based Care area. This MnCHOICES Certified Assessor position is part of the MnCHOICES Unit and works under the direction of the Home and Community Based Care Supervisor. The role conducts initial and annual assessments using the MnCHOICES Revision application and contributes to a collaborative team focused on innovation and person-centered care. After successful completion of the probationary period, the position is eligible for partial remote work.

Requirements

Bachelor of Arts or Bachelor of Science degree in Social Work, Psychology, Sociology, or a related area.
At least 2 years of previous experience in human or social services.
Valid Minnesota driver's license or evidence of equivalent mobility.
Successfully complete and maintain eligibility to provide direct care to clients in accordance with Minnesota Statutes §245C.03.

Responsibilities

Perform client screening and intake functions and activities for the division or program area.
Conduct activities associated with treatment planning, evaluation, and assessment.
Provide professional social and human services to clients, their families, and/or the community.
Perform case management requirements, documentation, and monitoring to meet mandates, document services, and address client needs.
Perform community relations, public relations, and outreach services in assigned program areas.
Collaborate with professionals, providers, therapists, health professionals, internal departments, law enforcement, community organizations, and government agencies to coordinate resources and meet client needs.
Perform other comparable duties as assigned.
