The Social Worker is responsible for participating in an interdisciplinary team approach to serve the needs of the Center population. This role provides therapeutic intervention to help residents cope with social and psychological aspects of their illnesses, impairments, or disabilities, as well as their feelings about institutionalization and separation from family, community, and loss. The Social Worker will maintain written documentation in the resident medical record, perform initial psychosocial needs assessments, and provide ongoing documentation for short-term residents. A key responsibility is developing the social service component of the resident's plan of care, identifying problems, goals, and approaches. The Social Worker will also participate in and coordinate interdisciplinary care planning conferences, ensuring resident and/or family input into the care planning process. This role acts as a liaison between residents, families, outside agencies, and the Center Administrator to uphold resident rights. Additionally, the Social Worker is responsible for conducting family meetings, facilitating discharge planning, ensuring the safety of Center customers, coordinating admissions through referrals, developing relationships with referral sources, verifying insurance coverage, and conducting facility tours.
