About The Position

Volunteers of America–Greater New York (VOA–Greater New York) is a leading anti-poverty organization serving neighbors in need across New York City, Northern New Jersey, and Westchester. Through 70+ programs, we support over 35,000 adults and children each year, including individuals experiencing homelessness, survivors of domestic violence, youth aging out of foster care, veterans, seniors, and children with developmental delays. Our staff are at the heart of this work, creating meaningful change every day. The Social Worker is creating and maintain a wellness and recovery-oriented environment by using person-first language, respectful documentation, person centered planning, and evidence-based practices to ensure the program provides comprehensive and integrated on-site social service that helps residents achieve their goals and successfully age in place. The Social Worker is responsible for the implementation of the treatment program in accordance with Volunteers of America policies and procedures, program models, funding source standards, and evidence-based practices. The Social Worker is responsible for meeting the highest possible standards of health and safety and provides all needed quality services to residents to meet contractual and regulatory requirements. The East Clarke Place Senior Residence program supports seniors who are chronically homeless who will also benefit from a comprehensive range of supportive services.

Requirements

  • Master’s degree in Social Work
  • two years of relevant experience working with individuals or families of domestic violence

  • LMSW is preferred

Responsibilities

  • Meet with residents to provide treatment, conduct psychosocial assessments, and develop individual service plans from intake through discharge in accordance with the program regulations.
  • Provide appropriate case management and therapeutic interventions focused on senior services including housing stability, medical and mental health stability, again in place, social isolation, etc.
  • Maintain case records and statistics for program.
  • Ensure support and education are provided and documented to assist residents in their transition into the community including referrals to appropriate social service agencies when the facility does not provide the needed service.
  • Provide ongoing risk assessments, crisis intervention and emergency response to address resident critical incidents.
  • Develop community linkages and refer resident to appropriate community providers to address their needs.
  • Work closely with hospitals and emergency rooms to coordinate mental health and medical services for residents.
