About The Position

Shriners Children’s St. Louis is seeking candidates for a PRN Social Worker to join our Care Management team. The Social Worker is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience. You'll help to provide continuity of care for positive outcomes in our family centered environment.

Requirements

• Social Work Masters
• SW License in State of Practice

Nice To Haves

• Case Management Certification (CM, ACM or ANCC)
• 3 Years of Social Work experience

Responsibilities

• Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.
• Coordinates the plan of care plan to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.
• Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
• Collaborates by working with patients, families, caregivers and the health care team locally and within the system to jointly communicate, problem solve, and share accountability for optimal outcomes.
• Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.
