Medical Social Worker Position Summary: We are looking for part-time Social Worker to provide case management and social work consultation within a multi-disciplinary team. Completes psychosocial assessments of patients and families to identify barriers to coping with acute or chronic illnesses/disabilities. Develops and implements a plan of care that empowers the patients/families to utilize resources necessary to ensure medical care and achieve optimal psychosocial functioning. Licensed Clinical Social Worker (LCSW) Position Summary: We are looking for a fulltime Licensed Clinical Social Worker to join the Complex Care Program to provide care coordination and co-management for children with medical complexity due to multiple chronic conditions. This role will meet with patients and families to identify barriers to coping with acute or chronic illnesses/disabilities, while also providing individual, group and family support to patients and their families enrolled the Special Needs program, both in an inpatient and outpatient setting. Provides crisis intervention as well as long-term therapeutic support for the entire family system.

Requirements

  • Master of Social Work (MSW) and Wisconsin Advanced Practice Social Work (APSW) certification/licensure required.
  • Minimum of one year of social work or related field experience.
  • Hospital experience preferred.
  • Crisis intervention and basic counseling skills.
  • Experience with the social, psychological and economic issues of children/families with severe, acute or chronic illness/disability preferred.
  • Knowledge of structure of the child welfare system, guardianship process and community pediatric resources preferred.
  • Knowledge of team process.
  • Excellent verbal and written communication skills.
  • Computer skills and the ability to learn the Children's database and electronic medical records, email and pager system.
  • Ability to function independently, prioritize and manage time.
  • Master’s degree in social work or related field, plus 3000 hours post Master's experience or PhD or PsyD from an APA accredited program.
  • State of Wisconsin Licensed Clinical Social Work (LCSW), Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT) licensure required.
  • Three or more years of experience working with pediatric patients.
  • Knowledge of psychotropic medications.
  • Ability to provide quality individual, group and family therapy.

Nice To Haves

  • Experience in working with children and youth with special health care needs and their families preferred.

Responsibilities

  • The role will assess, plan, and implement support plans to meet the overall needs of the entire family.
  • The role empowers patients/families to utilize resources to ensure necessary medical care and to achieve optimal psychosocial functioning.
  • The role will implement and maintain supportive programing within Complex Care to serve the entire family, including but not limited to sibling groups, educational opportunities, inpatient groups and family events.
  • This role will also assist with management of the Family Leadership Committee as well management and educational support of Trauma Informed Care within the Complex Care Program.
