About The Position

Nexus Children’s Hospital – Dallas is a specialized pediatric facility dedicated to caring for children with complex medical and behavioral health needs. Our interdisciplinary teams work collaboratively to provide compassionate, coordinated care that supports patients and families from admission through discharge—helping every child transition safely back to home and community. The Social Worker is a key member of the interdisciplinary team, supporting patients and families through the continuum of care with a strong focus on psychosocial support and discharge planning. This role builds meaningful relationships with patients and families, identifies barriers to successful discharge, and ensures clear communication between the care team and family throughout the hospital stay. The Social Worker plays a critical role in advocating for patient needs and facilitating safe, effective transitions to the next level of care.

Requirements

  • Bachelor’s Degree in Social Work (BSW) required
  • Minimum of 2 years of experience as a Social Worker or Case Manager
  • Current and active Social Work license in the state of Texas required

Nice To Haves

  • Master’s Degree in Social Work (MSW) preferred
  • Experience in a hospital or medical setting preferred

Responsibilities

  • Complete psychosocial assessments, social service screenings, and discharge planning assessments within 72 hours of admission
  • Develop and implement individualized social service care plans
  • Integrate social work plans into the interdisciplinary care plan and update regularly
  • Document all assessments, plans, and progress clearly in the medical record
  • Provide emotional support and counseling to patients and families
  • Build trusting relationships through empathetic, patient-centered interactions
  • Identify stressors impacting the hospital experience and communicate needs to the care team
  • Educate families on care expectations and discharge processes
  • Collaborate with physicians, case managers, and clinical staff to coordinate care and discharge plans
  • Facilitate and participate in family conferences and interdisciplinary team meetings
  • Identify discharge barriers early and escalate as needed
  • Assist in determining appropriate level of care post-discharge
  • Serve as a patient and family advocate within the care team
  • Communicate team recommendations and care plans clearly to families
  • Ensure continuity of care through effective communication with internal and external providers
  • Identify and coordinate appropriate community resources and referrals
  • Provide families with both verbal and written referral information
  • Maintain relationships with community providers and support services
  • Initiate CPS referrals when appropriate and support early intervention efforts
  • Ensure compliance with hospital policies, regulatory requirements, and documentation standards
  • Support safe discharge practices and patient safety initiatives
