About The Position
Affirmed Home Care is hiring a Full-Time Licensed Social Worker to support clients throughout Manhattan. This is a rewarding opportunity for a compassionate professional who wants to make a real impact by helping individuals and families safely age in place. In this role, you will provide client-centered social work services, support families, coordinate resources, and ensure clients receive the care and support they need to maintain independence and quality of life.
Requirements
- Active New York State LMSW or LCSW license
- Master’s Degree in Social Work (MSW)
- 1+ year of experience in healthcare or social services (home care preferred)
- Strong case management and care coordination skills
- Excellent communication and interpersonal skills
- Ability to manage caseloads in a fast-paced environment
- Compassionate, patient-focused approach to care
Responsibilities
- Conduct psychosocial assessments of clients and families
- Perform home visits to evaluate living environments
- Develop and implement individualized care plans
- Serve as the main point of contact for clients and families
- Coordinate community resources and support services
- Lead client and family meetings
- Collaborate with clinical and interdisciplinary teams
- Educate families on care plans and available resources
- Ensure accurate documentation and compliance
