About The Position

Suncrest Hospice is seeking a Social Worker to provide compassionate psychosocial support to patients, families, referring sources, and team members as they navigate the emotional, social, and practical challenges of serious illness and end-of-life care. The Social Worker serves as an advocate, resource, and trusted member of the interdisciplinary hospice team, helping patients and families receive support throughout their hospice journey.

The position includes assessing patients’ and families’ needs, providing counseling and emotional support, connecting them with community resources, advocating for their preferences and goals, collaborating with the care team, and maintaining documentation in accordance with hospice standards and regulatory requirements.

Requirements

• Master of Social Work degree that complies with CHAP and state-of-residence regulations.
• Valid driver’s license and car insurance.
• Reliable vehicle and willingness to drive.

Responsibilities

• Assess patients’ and families’ psychosocial, emotional, spiritual, and socioeconomic needs, and incorporate findings into the individualized plan of care.
• Counsel patients and families, providing emotional support, crisis intervention, and guidance through illness, loss, grief, and significant life transitions.
• Educate patients and families about available community resources and connect them with appropriate programs, services, and support.
• Advocate for patients and families, identify barriers to care, and work with the hospice team to develop appropriate solutions that address their needs, preferences, values, and goals.
• Participate in interdisciplinary team (IDT) meetings and collaborate with physicians, nurses, chaplains, aides, facility staff, and other care team members.
• Maintain accurate, thorough, and timely documentation in accordance with hospice standards and regulatory requirements.

• Salary of $75,000–$85,000
• 144 PTO hours and 40 sick hours per calendar year
