About The Position

Hartgrove Behavioral Health System is a leading psychiatric hospital with 160 beds, dedicated to providing quality behavioral health services to a diverse population of all ages. As a flagship facility within Universal Health Services, Inc., Hartgrove offers state-of-the-art technology and programs in the behavioral health field. We are committed to a teamwork approach, fostering a compassionate and therapeutic environment, and providing a continuum of specialty programs across inpatient, partial, and outpatient services. We are currently seeking to expand our Social Services Team and are offering a sign-on bonus for Bi-lingual Spanish speaking candidates.

Requirements

  • Master’s Degree in Social Work required.
  • Prior experience working with a psychiatric mental health population, preferably in an inpatient setting.
  • LSW, Illinois Clinical Social Work License preferred (LCSW).
  • Knowledge of case management, discharge planning, group leadership skills, age specific growth and development, crisis and behavior management, family therapy theory and practice.
  • Knowledge of theory and practice of psychosocial assessments.
  • Ability to work cooperatively with other hospital staff and involved community agencies.
  • Good organization skills, as well as oral and written communication skills.
  • Bi-lingual Spanish required.

Nice To Haves

  • Prefer knowledge of case management, discharge planning, group leadership skills, age specific growth and development, crisis and behavior management, family therapy theory and practice.
  • Illinois Clinical Social Work License preferred (LCSW).

Responsibilities

  • Conduct social history assessment interviews and produce social work assessments or updates based on interview data.
  • Document psychosocial histories, discharge planning, patient contacts, and treatment updates in the medical record.
  • Communicate essential information obtained through assessment to the assigned team member.
  • Initiate and follow through on discharge/aftercare planning as assigned.
  • Identify appropriate referrals for post-discharge services and establish rapport with referral agencies.
  • Consider basic growth and development milestones with interventions specific to the age and population served during assessment, treatment planning, and continuum of care processes.
  • Adhere to all policies and procedures regarding release of information and confidentiality.
  • Attend multi-disciplinary treatment staffings as assigned.
  • Function as a liaison between team members and all involved Social Service Agencies.
  • Regularly attend clinical supervision.
  • Conduct unit groups.
  • May provide clinical supervision for social work interns.
