The Social Worker will provide social work services to individuals or groups, including screening, psychosocial assessment, discharge planning, determination of social work goals, and appropriate interventions and follow-up. This role requires exercising professional judgment to recognize deviations from usual patient/family functioning, anticipate problems, and maximize coping capabilities. The Social Worker will conduct initial assessments, establish and follow care plans, and provide ongoing assessment of patient/family needs. Responsibilities include planning for discharge, implementing interventions, coordinating with healthcare team members, and collaborating with community agencies. Crisis intervention, documentation, and managing utilization of resources are also key aspects of this role. The Social Worker will educate patients/families regarding Advance Directives and assist with their execution, and communicate effectively with patients, families, healthcare teams, and community agencies to resolve issues impacting continuity of care. They will also provide support to patient's family members in the case of a patient's death, establish effective professional relationships, and adhere to the profession's Code of Ethics. The role requires knowledge and skills appropriate for the age of patients served, including understanding of growth and development over the life span.
