Join our Population Health team as a remote Social Work Case Manager and make a meaningful impact in the lives of elderly and disabled members. Through telephonic outreach, care coordination, advocacy, and connecting members to community and healthcare resources, you will support members with complex medical, behavioral, and social needs while empowering them to achieve their goals. In this role, you will partner with members, providers, caregivers, and community organizations to address barriers to care and improve health outcomes. To be successful in this role, you must hold an active Tennessee Social Work license and demonstrate strong communication, organization, and care coordination skills. We are particularly interested in candidates with experience in skilled nursing facilities (SNF), acute inpatient settings, discharge planning, community resource coordination, or hospice care.
