About The Position

BRAND Hospice is seeking a dedicated Licensed Master Social Worker (LMSW) to join their team as a patient-centered professional. The Licensed Medical Social Worker (MSW) will be primarily responsible for delivering exceptional social work services to hospice patients and their families, providing counseling and guidance to support their emotional well-being and coping capacity. This role is crucial in making a profound difference in patients' lives during challenging times. Requirements

• Master of Social Work from an accredited school of social work
• Licensed as a Social Worker from the State Board of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle
• Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred
• Current unencumbered license as a Social Worker from the State Board of Social Work (for AL, AR, GA, IL, MA, ME, NJ, NM, OK, OH, RI, SC, & TX)

Nice To Haves

• Minimum one (1) year experience as a social worker in a hospice setting

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement personalized social work plans and collaborate with your Bereavement Coordinator.
• Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
• Provide empathetic counseling to patients and families.
