About The Position
Responsible for providing ongoing psychosocial assessment, treatment and discharge planning to a variety of patients within the medical setting. Provides clear communication across the continuum to improve patient outcomes. Responsible for an assigned caseload of patients.
Requirements
- Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Master's Degree in Social Work field.
- Licensure as a Social Worker by the State of New Jersey required.
