About The Position

Join our team as a Social Worker. In this role, you will be responsible for providing social work services to the residents and family members of Continuing Care. Schedule: PRN/Flex arrangement to support the team with covering vacation, sick time, or medical leave absences from full-time team members during typical office hours. Compensation: $40.00 hourly.

Requirements

• Social Work degree (BSW or MSW) required, or minimum education requirements as per state licensure.
• Minimum of 3 years’ experience in working with the older adult population in post-acute/ long term care preferred.
• Knowledge of geriatric population and dementia.
• Specific state regulations and requirements may be applicable. These regulations take precedence over the requirements outlined in the job description.

Responsibilities

• Completing admission assessments, MDS, and other required documentation in accordance with State and Federal regulations
• Educating residents/families of the importance of advance directives and facilitating completion, signature, and tracking of documents
• Coordinating the Social Work function from admission to discharge to include room changes and transition planning
• Providing psychosocial support to residents and families to ensure a successful transition to Continuing Care
• Participating in care/service plan meetings with family members
• Providing bereavement support to residents, family members, and staff
