Requirements

  • Master’s Degree in Social Work (MSW)
  • New York State License as a Licensed Masters Social Worker (LMSW)
  • If a new graduate Social Worker, Limited Permit (must supply exam date prior to start of employment)
  • If a new graduate Social Worker, must have Licensed Masters Social Worker (LMSW) by the State of New York within 1 year of hire

Nice To Haves

  • At least three years of social work experience
  • New York State license as a Licensed Clinical Social Worker (LCSW)
  • SIFI certification preferred
  • Experience working with individuals and families affected by mental illness.
  • Experience providing evidence-based psychotherapeutic approaches.
  • Bilingual: English and Spanish speaking preferred

Responsibilities

  • Completes psychosocial assessments, appropriate behavioral health and disease-specific screeners, as well as other clinical duties within the organization's service standards and in an independent manner with appropriate use of supervision
  • Provides evidence-based behavioral health interventions and psychoeducation.
  • Assesses and systematically collects and tracks data in a patient registry related to patient behavioral health status and works with the care team to develop the plan of care.
  • Participates in-person or virtual huddles when shared patients are discussed.
  • Utilizes data, including registries, to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers.
  • Develops and maintains a working knowledge of community resources and managed care review processes.
  • Assumes responsibility and accountability for effectively managing behavioral health care of individual patients.
  • Documents history, assessments and interventions to reflect comprehensive and integrated approach to patient care.
  • Ensures complete, accurate and timely documentation in the electronic medical record.
  • Facilitates support groups with the goal to ensure patients receive adequate psychoeducation on their illness, as well as effective strategies for coping
  • Identifies learning needs of the patient and families, provides patient education; educates by adapting to patient and family learning style utilizing the teach-back method.
  • Attends team meetings and case conferences to present and discuss patient disposition and outcomes data.
  • Assists with the coordination of care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) serving as a key person to connect patients with resources in the community ensuring patients receive appropriate services via referrals and linkage
  • Assists with management of urgent and emergent patient care situations.
  • Assists with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness.
  • Provides care that is safe, ethical, patient- and family-centered, culturally congruent and evidence-based.
  • Performs other related duties as assigned and as appropriate to scope, education, and/or training.
  • When qualified, provides supervision to graduate social work student interns (SIFI eligible)
