About The Position

This role involves working collaboratively with property management and other personnel to support residents. The Social Worker will solicit and respond to referrals from property management, residents directly, and community members acting on behalf of tenants facing challenges with daily functioning. Key responsibilities include client assessments, developing and overseeing case management plans for long-term success, and collaborating with community agencies. The position also involves assisting residents with referrals to community resources, benefit programs, and related services, completing evaluations, plans, and service agreements when appropriate. Documentation of all contacts and activities in the SHP database is required. The role may also include accompanying residents to appointments, providing crisis intervention, counseling, case management, referring to long-term therapy, and assisting with property activities related to Health and Community Services functions. Other duties may be assigned by the Clinical Supervisor and/or Director of Resident Services.

Requirements

  • Ability to handle sensitive information in a confidential manner.
  • Excellent communication and organizational skills.
  • Computer skills, including Microsoft Word, Microsoft Outlook and Excel.
  • Ability to work with different cultures, ethnicities and minorities.
  • Ability to identify mental health conditions, diagnose and provide treatment and/or appropriate referrals.
  • Master’s Degree in psychology, social work or other relevant field
  • Valid CA Driver’s License with access to reliable vehicle.

Responsibilities

  • Working collaboratively and cooperatively with property management staff and other personnel
  • Soliciting and responding to referrals from property management staff, residents directly and people in the community calling on behalf of rental property tenants who have challenges related to day-to-day functioning.
  • Client assessments, development and oversight of case management plans for long term success.
  • Collaboration with other community agencies.
  • Assisting residents with referrals to community resources, benefit programs and related services.
  • Completing an evaluation, a plan and a service agreement with the resident when appropriate.
  • Documenting all face-to-face contacts, phone contacts with and on behalf of the resident and all activities related to the service agreement.
  • Maintaining identified service record in the SHP database.
  • Accompanying residents to appointments as required, to ensure services are received
  • Crisis intervention, counseling and case management with residents as needed.
  • Referral to long term therapy services in the community as needed.
  • Assisting with property activities related to Health and Community Services functions
  • Other duties as assigned by the Clinical Supervisor and/or Director of Resident Services
