About The Position

Maple Crest Health Center, part of American Baptist Homes of the Midwest, is seeking a compassionate Social Worker to join their team. This role focuses on providing emotional and psychosocial care to residents and their families, supporting them through various life transitions and health challenges. The Social Worker will be an integral part of a collaborative interdisciplinary care team, guided by ABHM values, and will contribute to a mission-driven environment focused on resident well-being across the continuum of care.

Requirements

• Must have a CSW certificate.
• At least 1 year of interdisciplinary team experience, with long-term care or hospice preferred.
• Strong communication, critical thinking, and documentation skills.
• Ability to work in a fast-paced, emotionally sensitive environment.
• Proficiency with computer systems and maintaining confidentiality.
• Ability to collaborate effectively with residents, families, and care teams.

Nice To Haves

• Long-term care or hospice experience preferred.

Responsibilities

• Conduct psychosocial assessments and provide counseling to residents and families.
• Support admissions, transitions, and adjustment to aging or illness.
• Collaborate with the interdisciplinary team on care planning and documentation (including MDS).
• Assist with discharge planning and coordination of community resources.
• Advocate for residents and provide crisis intervention as needed.
• Maintain accurate documentation and support financial/insurance processes.
• Serve as a liaison with families, agencies, and mental health services.
