About The Position

Centennial Peaks Hospital, located in Louisville, CO, has been providing psychiatric services to the Denver-metro area for over 30 years. It is a 104-bed facility offering a peaceful setting for treatment of mental illness, including chemical dependency, with unique and individualized programming for adolescents and adults. The hospital provides Electroconvulsive Therapy, Intensive Outpatient, Partial Hospitalization Programs, and inpatient services. Social Workers at Centennial Peaks provide clinical social work services across five units (general adult mental health, adolescent, two intensive treatment units, and a detox/dual diagnosis unit). They practice from a trauma-informed, strengths-based perspective, working with patients and their support systems to ensure success during and after hospitalization. This is described as a dynamic, fast-paced position with opportunities for growth and development, requiring a self-motivated, organized individual comfortable with multitasking and crisis management. Requirements

  • Ability to multi-task and thrive in a fast-paced environment is critical.
  • Master's degree in Social Work, Clinical/Counseling Psychology or Social Services related field REQUIRED.
  • License eligible degree required; must be able to register with DORA and be willing to work towards LCSW, LMFT or LPC if not already licensed.
  • Applicant must be available for a required minimum of 4 (four) shifts per month.
  • Applicant must be available for 2 weekend shifts a month required.

Nice To Haves

  • Licensure preferred.
  • 1-2 years mental health experience preferred.
  • Prior experience working with a psychiatric mental health inpatient setting preferred.
  • Knowledge of case management, discharge planning, group leadership skills, age specific growth and development, crisis and behavior management, family therapy theory and practice is preferred.
  • Social Workers must demonstrate flexibility for reassignment to units providing care to varied populations including, adults, adolescents, and dually diagnosed patients in an inpatient hospital setting.

Responsibilities

  • Provide patients and families with assessment, interventions, support, advocacy, and case management as a primary provider and/or as ancillary support to the primary medical provider.
  • Practice as a member of multi-disciplinary team, in the provision of clinical intervention and support to patients and families.
  • Assess, develop treatment plans, and intervene to address the needs identified in the assessment and by the Team.
  • Utilize knowledge of culture, age, and developmental stages to develop treatment plans and provide interventions, either as an independent practitioner or as a member of a clinical team.
  • Provide psychosocial assessments.
  • Lead daily process groups.
  • Participate in daily treatment planning.
  • Process mental health certifications.
  • Provide discharge planning/case management services with referral sources and other departments within the hospital.
