About The Position

The Social Worker assesses psychosocial needs and applies interventions to support the health treatment plan and address needs. The social worker applies interventions to enhance patient and family functioning and emotional well-being and facilitates access to appropriate services. This position is responsible for collaborating and coordinating care to enhance the patient experience and the achievement of collaborative goals through developing patient goals, monitoring progress, addressing barriers to goal achievement, and evaluating outcomes of individualized care plans. This role also provides training, guidance, and mentorship to home health colleagues and other providers on management of complex psychosocial patient issues.

Requirements

  • State of Wisconsin Driver’s License
  • State of Wisconsin Professional licensure for Medical Social Worker
  • Graduate degree in Social Work is required.
  • A minimum of 2 years of recent experience in healthcare social work is required.
  • CPR/BLS Certification required.

Nice To Haves

  • Experience and strong knowledge base in home health social work preferred.

Responsibilities

  • Evaluates complex patient and family situations which have legal, ethical, and psychosocial components.
  • Collaboratively develops patient goals, monitors progress, addresses barriers to goal achievement and evaluates outcomes of individualized care plans.
  • Assessments reflect interdisciplinary collaboration, which are comprehensive in relation to the patient’s age, diagnosis, support systems, and extent of intervention anticipated.
  • Demonstrates and applies knowledge of State and Federal regulations that ensure safety, quality of life, and protection of patient rights.
  • Participates in discharge planning for patients requiring post-discharge services such as transitions to skilled nursing facilities, assisted living facilities, outpatient dialysis, Acute Rehab, hospice, and transportation needs.
  • Engages patient/family in the treatment and decision-making process with respect to patient rights, principles of confidentiality, respect for patient privacy, and right to self-determination.
  • Provides age specific education and information to patient/family, particularly related to coping, community resources and advocacy.
  • Provides interventions which demonstrate knowledge of, and sensitivity toward cultural diversity and the spiritual, developmental, and educational backgrounds of the population served.
  • Demonstrates professionalism and good interpersonal skills in communicating with all customers. Utilize negotiation skills, which effectively promote constructive solutions.
  • Respects other professions and seeks opportunities for collaboration.
  • Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
  • Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
  • Focuses on improving processes and champions efficiency in daily work activities.
  • Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization’s occurrence reporting system and participating in developing improved safety processes, as appropriate.
  • Performs other duties as assigned.
