About The Position

This role involves providing teletherapy services within a school setting. The Social Worker will assess and treat patients referred by school staff, parents, PCP offices, and self-referred patients. Responsibilities include providing individualized therapy, monitoring patient progress, developing care plans, and making recommendations to various stakeholders regarding mental health treatment strategies. The role also requires responding to urgent mental health concerns, conducting suicide risk assessments, and assisting in connecting patients to appropriate care levels. Collaboration with greater Ambulatory Care Behavioral Health teams and participation in professional development, including peer case review and continuing education, are also key aspects of the position. Other job duties as assigned.

Requirements

• Master's Degree in Social Work
• 2 years of relevant health care related experience post obtaining Master's Degree in Social Work

Responsibilities

• Assess and treat patients referred by school staff, parents, PCP offices, and self-referred patients.
• Provide individualized therapy services and monitor patient progress.
• Develop individual plan of care and give recommendations to school staff, parents, PCPs, specialists, and other members of the interdisciplinary care team regarding mental health treatment strategies and identifies approaches to maximize continuity of care across the continuum.
• Respond to urgent mental health concerns, including suicide risk assessments and assist in connecting patient to appropriate level of care based on clinical assessment.
• Assist with greater Ambulatory Care Behavioral Health teams work as assigned.
• Participate in professional development opportunities through peer case review and monthly continuing education opportunities.
• Other job duties as assigned.
