About The Position

This position is responsible for providing comprehensive psychosocial assessments, coordinating services, and facilitating communication to support residents' emotional, physical, and psychological well-being and independence. The role involves meeting with residents and families, participating in care planning, completing documentation, and providing supportive counseling to residents. Additionally, the Social Worker will coordinate referrals to outside resources, assist with department supplies, attend meetings, and engage in professional development.

Requirements

Bachelor's degree in social work (BSW) or another related health field.
One year of related experience.
Advanced knowledge of a specialized or technical field, or thorough knowledge of the practices and techniques of a professional field.
Ability to read and interpret safety rules, operating and maintenance instructions, and procedure manuals.
Ability to write routine reports and correspondence.
Ability to speak effectively before groups of customers or employees.
No professional social work license is required for this staff Social Worker position.

Nice To Haves

Master's degree in social work (MSW) preferred.
Bilingual English/Spanish preferred but not required.

Responsibilities

Respond to resident call bells and refer resident and family requests to appropriate healthcare team members.
Interview residents and family members to obtain information for comprehensive psychosocial assessments.
Coordinate and monitor available services for individual residents to support an optimum level of emotional, physical, and psychological well-being and independence.
Identify and resolve problems regarding room changes, transfers, and discharge with appropriate departments and administration.
Facilitate communication with other departments on behalf of residents, including medical, nursing, dietary, rehabilitation, and psychiatric services.
Meet with residents and families and initiate discharge-planning activities.
Participate in Comprehensive Care Plan Meetings and team conferences and formulate comprehensive care plans.
Complete documentation in accordance with departmental requirements.
Document referrals received for resident intervention in a timely manner.
Consistently follow nursing home and departmental policies in the performance of duties.
Provide supportive counseling and mental health support to residents only. Provide family members with appropriate information, support, education, and referrals; mental health counseling is not provided to family members.
Participate, when requested by residents, in the ongoing functioning of the resident and family council.
Initiate and facilitate meetings among residents, families, and staff to promote a fuller understanding of institutionalized residents and joint participation in improving residents' emotional and physical well-being.
Coordinate and facilitate referrals for residents to outside resources for services not available at the facility.
Assist in the care and maintenance of department equipment and supplies.
Attend meetings as required and participate in committees as directed.
Enhance professional growth and development through educational programs, current literature, in-service meetings, workshops, and similar activities.
