As a hospice social worker, you will provide emotional and logistical support to patients and their loved ones during end-of-life care. You will help patients put their affairs in order in accordance with their wishes, including finances and funeral arrangements, and support families as they navigate grief. Most of your time will be spent caring for hospice patients wherever they call home, including private residences and assisted living or skilled nursing facilities. About once a week, you will meet at the office with the interdisciplinary team, including RNs, physicians, chaplains, nurses, and aides. The role works collaboratively with clinical teammates to provide compassionate, personalized care that honors each patient’s wishes.
