About The Position

Winning Wheels is accepting applications for a Social Worker to join our one of a kind atmosphere! Winning Wheels is an 88 bed non-profit rehabilitation facility, specializing in young adults with traumatic brain injuries, and spinal cord injuries. Requirements

• Must have a bachelor's degree, or equivalent in social work or related field, or be a Licensed Social Worker.

Responsibilities

• Develop, implement, and review care plans designed to meet the psychosocial needs of the resident. Complete quarterly assessments, and necessary MDS portions as well.
• Maintain ongoing communication with residents’ families, to ensure families are aware of changes, updates, etc., regarding their loved ones care.
• Participate in discharge planning, and navigating resources/referrals appropriate and needed for the resident.
• Perform administrative requirements, such as completing daily documentation, necessary forms, reports, etc., and submitting such to the Director as required.
