About The Position

The Social Worker will conduct formal intake interviews, make assessments and referrals, and complete psycho-socials and social histories. This role involves providing short and long-term counseling, crisis intervention, safety planning, and psycho-education. The Social Worker will also maintain accurate programmatic paperwork and statistical information, assist clients with entitlement forms, and make referrals for various services such as food, clothing, bathing, psychiatric and medical evaluations, and TB and HIV testing. Additionally, the role includes accompanying clients to appointments and handling follow-ups, making referrals to other service areas within Project Hospitality and the community, participating in weekly supervision and staff meetings, facilitating community meetings/groups and case conferences, and transporting/accompanying clients to appropriate referral sites when necessary.

Requirements

  • Masters Degree in Social Work
  • LMSW required
  • Minimum of 1-year experience working with the mentally ill and homeless populations
  • Experience with group work
  • Experience with individuals
  • Knowledge of DSM V
  • Exceptional communication skills both orally and in writing, with individuals and groups regarding confidential and sensitive issues.

Nice To Haves

  • Experience with substance abuse
  • Bilingual Spanish/English is a plus

Responsibilities

  • Accompany clients to appointments and handle follow-up.
  • Make referrals to other service areas within Project Hospitality and the community.
  • Participate in weekly supervision and staff meetings.
  • Facilitate community meetings/groups and ongoing case conferences.
  • Transport/Accompany clients to appropriate referral sites when deemed necessary.
