Social Worker Position – Eastern Panhandle

About The Position

This position provides social work services to patients, their families, and/or significant others through counseling, emotional support, assisting with environmental needs, crisis intervention, and group leadership. The role will work closely with primary care practices and the Population Health team to provide patient-centered support, connect individuals with appropriate community resources, and help address social needs that may impact their health and well-being. Although this position is remote, candidates who reside in or are familiar with the Eastern Panhandle of West Virginia, particularly Berkeley and Jefferson counties and the surrounding communities, are preferred. Familiarity with the region and its community resources will be valuable in supporting local primary care practices and the patients served.

Requirements

  • Master's degree in social work or related field
  • Current social worker licensure as required by the state where services are being performed: WV: Licensed Graduate Social Worker (LGSW), Licensed Certified Social Worker (LCSW) OR Licensed Independent Social Worker (LICSW) through the West Virginia Board of Social Work
  • PA: No state licensure required
  • OH: Licensed Social Worker (LSW), Licensed Independent Social Worker (LISW), OR Licensed Independent Social Worker-Supervision (LISW-S) through the Ohio Board of Social Work
  • MD: Licensed Masters Social Worker (LMSW), Licensed Certified Social Worker (LCSW), OR Licensed Certified Social Worker-Clinical (LISW-C) through the Maryland Board of Social Work
  • Candidates must hold a master’s degree in social work and have an active West Virginia social work license at the time of hire.
  • Candidates will also be required to obtain master’s level social work licensure in Maryland within six months of hire.
  • Excellent verbal and written communication skills.
  • Ability to work collaboratively with health care professionals at all levels to achieve established goals and improve quality outcomes.
  • Basic computer knowledge and ability to operate standard office software.

  • PA: Licensed Social Worker (LSW) OR Licensed Clinical Social Worker (LCSW) through the Pennsylvania Board of Social Workers, Marriage and Family Therapists and Professional Counselors
  • Two (2) years' social work experience.

Responsibilities

  • Provide psychosocial assessments of patients and families to identify emotional, social, and community strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
  • Participate as an integral professional member of an interdisciplinary treatment team led by a physician.
  • Formulate, develop, and implement comprehensive treatment plans utilizing appropriate and evidence-based treatments and interventions, which may include crisis intervention and solution-focused brief intervention.
  • Screen, educate on diagnosis, and provide therapy and interventions to individuals with a variety of presenting problems including substance abuse, depression, anxiety, thought disorders, trauma, and autism spectrum. May perform these functions independently or as part of a team (if under the supervision of LICSW).
  • Provide consultative services to health care team members within scope of care definitions as needed.
  • Maintain a working knowledge of relevant medical/legal issues that impact patient care, e.g., advance directives, child and elder abuse.
  • Provide education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment, and/or life situation.
  • Participate in multi-disciplinary health care teams and represent the clinical social work perspective/liaison with patient and family.
  • Formulate, develop, and implement discharge plans beginning on the day of admission through length of stay.
  • Act as a liaison with community agencies and resources to assist patients and families as needed.
  • Facilitate cognitive/psychoeducational group therapy at a minimum of once per day and provide individual Solution Focused Brief therapy and crisis intervention as ordered by doctor and/or as needed.
  • Provide updates and information to families/support persons and care providers throughout the patient's stay and facilitate family meetings to provide discharge planning and education to family, as applicable.
  • Facilitate referrals to placement options including but not limited to personal care homes, nursing homes, skilled rehabilitation, assisted living, and group homes.
  • Document assessment, plan, interactions, and interventions according to departmental, hospital, and/or health system guidelines and standards.
  • Maintain electronic medical records and documentation in accordance with department, hospital, medical center, and/or health system policies.
  • Attend and participate in staff, committee, department, and other administrative meetings.
  • Complete annual required education related to specialty accreditation as defined by accreditation standards (may be completed through CBLs, trainings, In-services, and competency validation).
